Fall 2008 Registration Form - Due July 29, 2008
- FEES: All Players:
Fairfax County Resident: $64.00* / $55** for GGM division (see acceptable proof of residency)
*** Out-of-County Resident: $84.00 /$75 for GGM division
Team Rep Deduction: - $5
* Registration fee includes Fairfax County's recently approved mandatory application fee.
** Registration fee now includes 10 games.
*** Out-of-county Fee Information: Fairfax County residents must provide valid proof of residency in order to avoid out-of-county fees.
- Dual registrants must pay this fee TWICE...that is, once for EACH team with which you are registering.
- DEADLINE & LATE FEES: After July 29, 2008, add late fee of $15.00 for returning FWSA players registering after 07/29/08. (NEW players to FWSA do not pay a late fee)
- DUAL REGISTRATION: 2nd team
requires separate payment AND second out-of-county fee.
- ACCEPTABLE PROOF OF FAIRFAX COUNTY
RESIDENCY:
- Driver's license
- DMV-issued ID card
- Voter registration card
- Active military ID
- Proof must match name and address on registration form, must be on an 8 1/2 X 11 sheet of paper with team name written on lower right corner.
Team Reps: To register a team, you need to submit:
- Two typed team rosters, forms, residency proofs, checks in large envelope to Division Commissioner.
- Organize forms and checks alphabetically, by last name.
- Keep forms and proofs together; checks in separate stack.
- Do not use staples, paper clips, or glue.
- PAYMENT: Make checks payable to FWSA. No cash will be accepted.
If a check covers more than one person, including the person writing the check:
- Write the full amount of the check in CHECK AMOUNT on the check writers form. Enter the check number in the CHECK # spot.
- For other players covered with this check place "0" in CHECK AMOUNT and same check number in CHECK #. List all players by last name in the lower left corner of the check as well as on the registration form.
If a check covers several players but not the person writing the check:
- Select one player at random and write full amount of check in CHECK AMOUNT on her form. Enter check number in CHECK #.
- For other players covered with this check place a "0" in CHECK AMOUNT and same check number in CHECK #.
List all players by last name in the lower left corner of the check as well as on the registration form.
When you have more than five checks to submit, please complete this form and include it with the registrations
- Team Representative gives checks and forms to Division Commissioner
**Incomplete registrations will be returned to the Team Rep and Player will not be registered!**
